All regions in the country have recorded positive COVID-19 cases yet almost half of the total number came from Metro Manila. However, according to the data from the Department of Health, there are still provinces that remain to be free from the virus as of April 21, 2020. These provinces are as follow:

- Apayao
- Aurora
- Basilan
- Batanes
- Biliran
- Camarines Norte
- Davao Occidental
- Dinagat Islands
- Eastern Samar
- Guimaras
- Ifugao
- Kalinga
- Masbate
- Mountain Province
- Quirino
- Sarangani
- Siquijor
- Sorsogon
- Southern Leyte
- Sulu
- Surigao Del Norte
- Surigao Del Sur
- Tawi-tawi
- Zamboanga Del Norte
- Zamboanga Sibugay
With the community quarantine being in place the spread of the virus was massively minimized which results for these provinces to be COVID-free.
